Position Summary:
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our home health team. As a vital part of our interdisciplinary care model, you will provide in-home physical therapy services to clients across our service area. This position offers a flexible schedule, a supportive clinical team, and the opportunity to make a lasting impact on the lives of those we serve.
Key Responsibilities:
Evaluate patients and develop individualized treatment plans in coordination with physicians and other care team members. Deliver hands-on physical therapy interventions in the patient’s home setting. Educate patients and caregivers on exercises and techniques to support recovery and independence. Document all care and patient progress accurately and in a timely manner. Participate in interdisciplinary team communication to ensure continuity of care. Maintain compliance with all regulatory and organizational standards.
Qualifications:
Current and valid Physical Therapist license in PA. Graduate of an accredited physical therapy program. Minimum 1 year of clinical experience preferred (home health experience a plus). Strong communication and organizational skills. Valid driver’s license and reliable transportation.
