Works under the supervision of a Clinical Manager. Providesindividualized patient care for patients in all developmentalstages throughout the life span including: Adult – 18-72 years,Geriatric – 72 + years, according to established policiesprocedures, and guidelines. Participates in professionalresponsibilities such as program development, student programs, andperformance improvement programs. Provides patient consultation andassumes communicative role with other professionals. Referspatients to other professionals as appropriate and ordered.
Performs other duties as assigned.
PRIMARY JOB DUTIES1.
Assesses,interprets, plans, implements, and evaluates patients withappropriate level of skill given the patient’s age and diagnosis.
Admits patients to the agency when P.T. is a primary skilledneed.2. Assists with program development and discipline specificoperations efficiently and productively.3. Appropriatelycommunicates and documents information in accordance to agencypolicies and procedures and discipline-specific guidelines.4.Contributes to program effectiveness.5. Education:
Graduate of Physical Therapy programfrom an accredited university.2. Licensure /
Certifications:
Licensure in the state in which providing therapy (NC/SC); CPRcertification.3.
Experience:
Minimum 1 year.4.
Essential Technical/ Motor Skills:
Knowledge of home health and physical therapyprocedures, ability to manipulate patient care and evaluationequipment, including modalities and assistive devices; ability toproperly transfer and guard patients; ability to apply other manualtherapy techniques.5.
Interpersonal Skills:
Ability to deal withthe public, co-workers, patients, families, and physicians in aprofessional and positive manner.6.
Essential PhysicalRequirements:
Requires significant locomotion, frequent liftinggreater than 50lbs and transferring patients greater than 100lbs.
Also repeated reaching in front of the body, bending and stooping,pushing and pulling. Occasionally requires reaching overhead, stairclimbing, sitting and fine motor manipulation.7.
