JOB PURPOSE
To provideevaluative, consultative and treatment interventions to all homehealth patients in their place of residence referred by a licensedphysician and will maintain effective communication with homehealth partners patients, patient’s families and other internal andexternal customers as appropriate.
KEY RESPONSIBILITIES
- Evaluatethe patient’s level of function and skilled therapy needs.
- Treatthe patient to relieve pain, develop or restore function, andobtain maximum outcomes according to the established plan of care.
- Advise and consult the family and other interdisciplinary teammembers to promote optimal functional outcomes.
- Assesses,monitors, documents and reports progress of patient’s health andcondition using required documents via clinician preferredtechnology, and within established timelines
- Helps maintaincontinuity of care by collaborating with the interdisciplinary teamand attending physician to develop, integrate, and manage thepatient care plan of care, making referrals to other disciplines asneeded
- Demonstrates intermediate knowledge of home care andcompetency in discipline for specific patient care skills, requiredfor the provision of care
- Conducts field visits to (1) evaluate ∈itiate start of care and (2) conduct clinical outcomesmonitoring, follow-up and care performance improvement initiatives
- Utilizes accepted company teaching materials in patient/familyeducation
- Completes documentation timely, accurately, and at thepoint of care, according to industry standards (e.g., OASISassessments, SOC/Admissions, Recertifications, ABN’s, subsequentvisits, physician orders, care coordination, etc.)
- Meets agencyproductivity standards and utilizes time and resources effectivelyand efficiently
- Participates in center/agency surveys(Licensure/Joint Commission), mandatory in-services, caseconferences, staff meetings, and programs (e.g., PerformanceImprovement Program) as required
- Follow established safetyregulations and procedures, and report job-related functions/tasksthat involve occupational hazards, as necessary
- Maintains currentlicense, certification, and clinical records to meet federal, stateand agency regulations and guidelines
- Recognizes and responds topriorities in patient care showing initiative and sound judgment.
- Demonstrates willingness to adapt to change in workload and assistswith training/orienting others.
- Accepts responsibility to reportany alleged or suspected violations to supervisor and/or othersuntil the issue is resolved.
- Ability to be self-directed,prioritize, manage time appropriately, and meet outcomes andstrategic goals by targeted deadlines.

LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS
- Must have avalid professional license in State of practice.
- Graduate of anaccredited Physical Therapy Program.
- If foreign-trained, theprogram must be deemed substantially equivalent to a PhysicalTherapist entry level education in the U.
S. by a credentialsevaluation organization approved by the
APTA. PREFERRED SKILLS ANDEXPERIENCE
- Prior Home Health experience is preferable.
