Job DescriptionAccept and manage patients within your designated geographical area of coveragePlan, prepare, and implement customized therapy programs designed to restore overall function and promote independence in the home settingDevelop and maintain individualized care plans/goals in collaboration with physicians, patients, and their familiesEnsure timely and accurate documentation of all patient visits and therapy servicesEffectively communicate discharge planning with patients, families, and interdisciplinary team membersMaintain compliance with all clinical and documentation standardsQualificationsPhysical Therapist Program Graduate.
TN Physical Therapist License or eligible.
Liability insuranceHome health experience is preferred but not required – We will provide you with a mentor to help with documentation and compliance.
