PRN Home Health Physical Therapist Position Available In Davidson, Tennessee

• Qualifications
• Graduate of physical therapyprogram.
• Possesses current registration or license as required bythe state of practice.
• Minimum of two (2) year experience in homehealth care or equivalent experience.
• Demonstrates good writtenand verbal communication skills, interpersonal skills.
• Licenseddriver with automobile insured in accordance with state/Agencyrequirements.
• Good driving record and reliable transportation.
• Current CPR certification.

• ESSENTIAL FUNCTIONS/AREAS OFACCOUNTABILITY

• Performs physical therapy assessments, diagnostictests, skilled procedures, and ongoing evaluation of clients whoare receiving services under a medically approved plan of care.
• Completes initial assessments in forty-eight (48) hours of referralunless other arrangements are made.
• Develops a therapy care planto provide direction to other health team members involved in theplan of care. Reviews and updates per Agency policy and clientneeds.
• Documents all findings, plans, interventions, and clientprogress (outcomes) on designed forms and submit per Agencyguidelines within forty-eight (48) hours.
• Communicates plans andchanges to the physician and to case manager and other Agency staffthrough the care plan, written progress notes, and participation incare conferences.
• Implements teaching appropriate to the clientsneeds and within the context of his/her environment and learningability.
• Plans, teaches, supervises, and counsels clients abouttheir physical therapy need to promote improvement or recovery,symptom management of disease process, and community resources.
• Evaluates the effectiveness of the teaching plan and establishescare plan for others to follow on teaching the client.
• Uses OASISassessment tool and outcome reports to improve therapy services andclient outcomes.
• Demonstrates knowledge of safety/infectioncontrol practices by compliance with agency policy and procedures.
• Assesses safety of environment and takes initiative to help preventaccidents and promote safety.

• TYPICAL PHYSICAL DEMANDS

• Requires afull range of body motion including the handling and lifting ofpatients.

Manual and finger dexterity and eye-hand coordinationrequired. Occasionally will lift and carry items weighing up to 50pounds or greater. Requires corrected vision and hearing to normalrange. May require working under stressful conditions or workingirregular hours. Involves exposure to communicable diseases.

Requires a current drivers license and the ability to drive andaccess a patients home without assistive device.

Pay:

$75.00 -$85.00 per hour
