Staff HomeHealth PT PPV Model Knox County The Registered Nurse is a licensedprofessional who provides direct and indirect patient care servicesto patients in their home environment or alternative settingaccording to the organization’s policies and procedures. The nurseassesses patients and develops a plan of care in collaboration withthe physician. The nurse provides nursing interventions (includingphlebotomy, wound care, infusion, ostomy, catheter care and otherskilled interventions) as ordered by the physician. The nurseinstructs, motivates, and assists patients and/or caregivers inlearning and improving the ability to live independently at home.

The nurse supervises and directs the efforts of the LicensedPractical Nurse and Home Health Aide when involved in the patientplan of care.

PPV MODEL

• 28 to 30 points/week $.

41/mile travelreimbursement SOC ROC Recert Evaluation Discharge Hourly rate of$35/hr for
