Job Summary and Qualifications ThePhysical Therapist (PT) provides high-quality, patient-centeredcare by managing the complete process of therapeutic care of anassigned group of patients in coordination with the physician andthe patient care team. The Physical Therapist completes patientassessments and participates in developing and implementingindividualized care plans focusing on goal achievement, patientengagement, and positive patient experience. The Physical Therapistprovides or directs a majority of the patient’s physical therapy tosupport patients throughout the continuum of care. What you will doin this role: You will provide evaluations and treatmentinterventions within the scope of practice established by law andprofessional code of ethics. You will assess and evaluatetherapeutic/rehabilitative/functional status and participate in thedevelopment of the total plan of care. You will provide purposefuland skilled therapeutic care at patient’s home, using proceduresand techniques to produce a change in condition/function consistentwith the physician’s plan of care. You will instructpatient/family/caregiver in the use of therapies, exercises,therapeutic appliances, and/or prosthetic devices to be usedoutside of therapy sessions. You will prioritize caseload,determine daily staffing needs and coordinate assignments/handoffs.
