Job Summary andQualifications The Physical Therapist (PT) provides high-quality,patient-centered care by managing the complete process oftherapeutic care of an assigned group of patients in coordinationwith the physician and the patient care team. The PhysicalTherapist completes patient assessments and participates indeveloping and implementing individualized care plans focusing ongoal achievement, patient engagement, and positive patientexperience. The Physical Therapist provides or directs a majorityof the patient’s physical therapy to support patients throughoutthe continuum of care. What you will do in this role: You willprovide evaluations and treatment interventions within the scope ofpractice established by law and professional code of ethics. Youwill assess and evaluate therapeutic/rehabilitative/functionalstatus and participate in the development of the total plan ofcare. You will provide purposeful and skilled therapeutic care atpatient’s home, using procedures and techniques to produce a changein condition/function consistent with the physician’s plan of care.

You will instruct patient/family/caregiver in the use of therapies,exercises, therapeutic appliances, and/or prosthetic devices to beused outside of therapy sessions. You will prioritize caseload,determine daily staffing needs and coordinate assignments/handoffs.

What qualifications you will need: Bachelor of Science in PhysicalTherapy from an accredited program Certifications, Licenses, andother Special Requirements Completion of the National CertificationExamination for Registered Physical Therapist License to practicephysical therapy within the state Ability to work varying hours,including weekends and holidays Must be a licensed driver with anautomobile that is insured in accordance with state and/ororganization requirements and is in good working order LHealthcareat Home is a service line from HCA healthcare.
