In this role, you will be responsible for planning and implementing individualized treatment programs for patients with various physical conditions. You will work in an outpatient setting, providing high-quality and hands on patient care to help individuals regain mobility and improve their quality of life. Duties Develop and implement personalized treatment plans based on patient evaluations. Utilize various therapeutic techniques, including acute care methods and trigger point therapy. Provide patient education on exercises and techniques to promote recovery and prevent injury. Collaborate with other healthcare professionals to ensure comprehensive patient care. Participate in discharge planning to facilitate smooth transitions for patients leaving the hospital or clinic. Maintain accurate documentation of patient progress and treatment outcomes in accordance with healthcare regulations. Stay updated on advancements in physical therapy practices and systems to enhance patient care. Skills Strong knowledge of anatomy and physiology relevant to physical therapy practices. Experience in outpatient orthopedics. Proficient in discharge planning processes to ensure effective patient transitions. Excellent communication skills for effective interaction with patients, families, and healthcare teams. Ability to provide compassionate patient care while maintaining professionalism. Familiarity with various therapeutic modalities and rehabilitation techniques. Strong problem-solving skills to address patient challenges effectively.
