Per Diem Job Summary:

We are seeking reliable and compassionate Physical Therapist Assistants (PTAs) for per diem roles providing in-home care. The ideal candidates will work under the supervision of a licensed Physical Therapist to help patients achieve their rehabilitation goals in compliance with Medicare home care standards.

Key Responsibilities:

Implement treatment plans developed by a licensed Physical Therapist. Assist patients with exercises and mobility techniques to improve strength, balance, and function. Observe and report on patient responses to therapy, making recommendations to the supervising PT. Instruct patients and caregivers on home exercises and safe movement strategies. Accurately document patient care activities and progress per Medicare guidelines. Communicate effectively with PTs, nurses, and the broader care team. Travel to patients’ homes Associate degree from an accredited Physical Therapist Assistant program. Current state licensure/certification as a Physical Therapist Assistant (PTA). CPR certification (required). 1 year of experience in home health or a clinical setting preferred. Knowledge of Medicare documentation requirements and home care best practices. Strong interpersonal and communication skills. Ability to work independently and manage time effectively. Reliable transportation and valid driver’s license.
