Licensed Home Care Physical Therapist Assistant (PTA)

• 5:00pm

Territory:

• -This position will work in the fieldhelping patients in their home setting.

The office is in-Roseville, MN and the general territory for this position would bethe northeast metro.

Pay and Benefits:

Hourly Pay Range:

• $29.00
• $37.

Two (2) years of experience as a PTA-preferred.

Able to travel within geographic area serviced by theoffice.

• license, reliable transportation, andproof of auto insurance.

Licensed Physical Therapist Assistant:

Participate in the development andevaluation of the plan of care.

Provides services that are orderedby the physician as indicated in the plan of care.

Assists thetreating Physical Therapist in documenting a patient’s progress andresponse to treatment.

Completes and submits timely, legible,accurate, relevant and sufficient documentation addressing thetreatment and interventions provided, the patient’s response to thetreatment and interventions, and the patient’s progress towardgoals in accordance with professional standards of practice, policyand procedures, and payer requirements.

Counsels and educates thepatient and the family, and other caregivers to promote patientprogress toward mutually established goals and in meetinghealthcare related needs.
