ESSENTIAL DUTIES

Supports activities as introducedby skilled nursing facility and RehabilitationServices.
Demonstrates superior customer service through courtesy,consistent follow-up and positive communication.
Utilizes continuingeducation assistance to achieve targeted qualityimprovements.
Provides ongoing collaboration with the supervisingPhysical Therapist, Rehabilitation Director and other disciplinesto facilitate effective and efficient clinical casemanagement.
SKILLS AND
ABILITIESAble to provide treatments whichmeet patient needs, including use of current treatment knowledge inaccordance with Rehabilitation Services guidelines and SkilledNursing Facility and/or Home Health reference guide asindicated.
Able to demonstrate Rehabilitation Servicesinter-disciplinary approach to patient care to facilitate effectiveand efficient clinical case management utilizing evidenced basedclinical practice.
Able to complete clear and accurate documentationin accordance with Rehabilitation Services, regulatory, licensing,payor and accrediting requirements.
MINIMUM QUALIFICATIONS
Atminimum, possess an Associates degree in Physical Therapy.
Possess acurrent, unencumbered, license to practice as a Physical TherapistAssistant in the state of Georgia.
Prefer two (2) years experiencein a skilled nursing facility rehabilitation environment, orequivalent.
