Key Responsibilities:
Patient Treatment & Support Provide hands-on therapy under the supervision of a Physical Therapist (PT). Implement individualized treatment plans to improve strength, balance, endurance, and mobility. Train patients in gait techniques, transfer strategies, and the use of assistive devices. Educate patients and caregivers on therapeutic exercises and home exercise programs. Collaboration & Communication Work closely with the PT, nursing staff, and interdisciplinary team. Document patient progress and communicate updates to the PT. Participate in care planning and discharge planning to ensure patient success. Educate caregivers and staff on mobility techniques and fall prevention. Compliance & Professional Development Maintain accurate and timely documentation in accordance with facility and regulatory guidelines. Ensure treatments align with Medicare and other reimbursement regulations. Participate in in-service training and performance improvement programs.
