Position Summary:

The primary purpose of your job is toreturn resident to optimal physical function and facilitateachievement of patient, physician, and therapy goals in a SNFsetting. Instruct the patient in maintaining achieved level offunction after discharge through education, independent exerciseprogram, and appropriate activity modification.

Education/Qualifications:

• Must have an Associate’s Degree in aPhysical Therapy Assistant program from an accredited college oruniversity.
• Must possess a current, unencumbered license topractice as a Physical Therapy Assistant within the state in whichyou work.
• Must possess basic computer skills and ability tonavigate through various software programs, including servicedelivery software and the electronic health record software.
