Overview Provides physical therapy interventions to designatedpatients under the supervision of a Physical Therapist (PT).Assists in monitoring patient progress through observations andfollows up with supervising therapist.   • Follows establishedphysical therapy plan of care, including physical therapyinterventions and patient/caregiver education. Modifies specifictreatment procedures in accordance with PT plan of care andPhysical Therapy Assistant (“PTA”) scope of practice. Documents allcare provided and maintain records per VNS Health policy andstandards.
  • Assists in monitoring patient progress throughobservation and follows up with Supervising Therapist in accordancewith VNSNY policy.
  • Complies with scope of practice for PTA,including a co-visit with Physical Therapist on all initial PTAvisits and keeps PT informed. PT will re-assess patient every 6visits or 30 days (whichever comes first).
  • Manages assignedcaseload; provides physical therapy services to patients asdirected per PT plan of care.
  • Participates in VNS Healthmeetings, including but not limited to, interdisciplinary teammeetings, in-service and training programs, rehab services meetingsand caseload reviews.
  • Travels to patients’ homes and/or otherfacilities with varying environments (e.g., elevated buildings,walk-ups, care facilities, single/multiple family homes, presenceof pets, etc.) using approved transportation options to deliverdirect care to the patient.
  • Assists patients with therapeuticinterventions and activities of daily living, which may requirepositioning, moving, transferring and lifting patients of varyingbody sizes and physical conditions, with and without assistance andas appropriate, instructing, demonstrating and verifyingcapabilities of family members and/or paraprofessionals.

Ability touse and assist patient with use durable medical equipment as acomponent of the plan of care, including but not limited towheelchairs, patient transfer lifts, ambulatory devices, slings,splints, orthoses, prostheses, and exercise/strengtheningequipment.

  • Provides care in accordance with Agency policies,practices, procedures and standards, including professionalpractice standards of care, which may require standing, stooping,sitting, crouching, twisting, reaching, bending and stretching todeliver patient care.
  • Transports and utilizes required technologydevices (tablet), infection control materials and supplies usingVNS Health designated/supplied carrying case (backpack) weighingapproximately 15
  • 20 lbs to and from patient homes/carefacilities, VNS Health offices and therapists’ home.
  • Participatesin special projects and performs other duties as assigned.
Qualifications Licenses and Certifications:
  • New York StateCertification as Physical Therapist Assistant Required
  • Validdriver’s license may be required, as determined byoperational/regional needs.
Education:
  • Associate’s Degree asPhysical Therapist Assistant from a New York State approved programor whose educational credentials have been approved equivalent by
New York State Required Work Experience:
  • Minimum of two yearsexperience in clinical patient care (per NY State Education Law)Required
  • Bilingual skills may be required, as determined byoperational needs.
