Your Responsibilities Will Include:

Implementing care plans under the supervision of a licensed Physical Therapist Documenting patient progress, treatment notes, and discharge summaries accurately and timely Assisting patients in performing therapeutic exercises and daily activities Promoting a safe, positive, and encouraging rehabilitation environment

Qualifications:

Valid New York State Physical Therapist Assistant License Experience in Skilled Nursing Facilities (SNF) and/or Acute Care is preferred Familiarity with Rehab Optima or other EMR systems is a plus Must be fully vaccinated against
