Home Health Physical Therapist Assistant PTA Full Time Position Available In Davidson, Tennessee

Job Description:

Position Overview We are looking for a Physical TherapyAssistant (PTA) to join our team! You will utilize your clinicalexpertise to assess, teach, and deliver therapy interventions andtreatment directed by the Physician as established in the homehealth plan of care. You will collaborate with interdisciplinaryteam members, receiving and providing pertinent updates to optimizepatient well-being.

Full Time visits needed Monday –

Friday Essential Job Functions:

Provides direct physical therapyaccording to directions of physical therapist and in accordancewith the physician’s plan of care. Communicates with and regularlyupdates the patient and/or caregiver regarding the plan of care.

Provides skilled therapy services, treatments and procedures asappropriate for the patient’s care and safety. Reports observationsof the patient’s condition to the designated supervisor(s), and/orthe patient’s physician, as appropriate. Treats clients to relievepain and/or develop or restore function to maintain maximumperformance. Provides skilled teaching to the patients, familymembers, and caregivers regarding the safe use of prosthetic,orthotic and assistive devices (i.e., canes, walkers, wheelchairs,etc.) Educates patients and caregivers in carry over regardingissues concerning rehabilitation and adaptation techniques neededfor recovery. Collaborates with other service disciplines todiscuss the patient plan of care. Plans for client’s discharge,prepares discharge instructions, and communicates discharge planwith patients, family members, and caregivers. Participates inin-service programs. Reacts to change productively and performsother job-related tasks and duties as assigned. Complies withethical and professional standards of conduct as set forth by theAmerican Physical Therapy Association Ensures confidentiality ofpatient records/information. Observes, records, and reports (1)signs and symptoms; (2) reactions to treatments and medications;and (3) any change in the patient’s physical or emotional conditionto the patient’s physician and to the RN on the case if applicable,on a timely basis. Records clinical and progress notes at the timeof the visit to ensure continuity of qualitative patient care on atimely basis. Completes daily report and any required documentationin a timely fashion to ensure agency operating schedules. Maintainsconsistent attendance. Submits all credentials in a timely manner.

Requirements:

Graduation from a Physical Therapist Assistanteducation program from an accredited university/program. Current,unrestricted state license as a Physical Therapy Assistant in thestate of practice

Valid CPR Preferred:
