Job Description

Overview Accordia Health in Rockford, AL is a Federally Qualified Health Clinic (FQHC) that assures that our patients receive the medical services they need through a caring, holistic approach to healthcare delivery. We coordinate treatment, medications, referrals, and any other aspect of our patients’ care required to help them maintain wellness. This position will be working in an approved site for loan repayment options via National Health Service Corps (NHSC) or Nurse Corps.

Responsibilities Clinical Responsibilities:

Provides medical evaluation and treatment of the patient Interviews patients to obtain history, performs physical examinations, orders lab and other tests, prescribes medications and treatments Provides continuity in managed care for patients with pre-existing long term problems. Makes referrals for secondary and tertiary care Performs medical procedures according to privileges issued Provides health maintenance visits, evaluates for immunizations and provides anticipatory guidance and referrals; performs contract and special physical examinations such as employment, etc.; performs STD (sexually transmitted disease) and family planning screening and education Documents and codes all patient contact accurately and legibly in the medical record Performs as an active member of the medical team Performs other duties as assigned

Supervision and Consultation:

Effectively collaborates with supervising Medical Doctor in accordance with the signed protocol and formulary Seeks clinical supervision and consultation as needed Accepts and employs suggestions for improvement Actively works to enhance clinical skills

Clinical Record Keeping:

Documents per AltaPointe policy in a timely fashion Documents appropriate, complete, and clinically comprehensive records per AltaPointe policy Documents in a clear and concise manner, patient symptoms and treatment

Case Load Management:

Effectively manages caseload based on patient needs Meets AltaPointe productivity standards Qualifications Completion of an accredited physician assistant training program and licensure with the state of Alabama to practice as a Physician Assistant.
