Mon-Fri In collaboration with attending surgeon, this roleis responsible for establishing a new clinical program whileproviding skilled health care to patients in a variety of clinicalsettings and facilitating clinical management of patients bypartnering with clinical nurses and other members of the healthcare team. The scope of responsibility is to assess, plan, evaluateand treat human responses to actual or potential surgical problemsutilizing appropriate practices, standards, protocols andguidelines. Provides direct patient care for in officeconsultations, including assessment, workups, triage, procedures,emergency work-ins, and other patient care activities Assistsattending surgeon in the operating room with colorectal proceduresObtains and documents health history, performs and documentsphysical exam, and orders and interprets diagnostic tests Providescounseling to patients regarding diagnosis and expected course oftreatment Collaborates with physicians to provide appropriatedirect care interventions within scope of practice Collects,organizes, records, and discusses data as appropriate and usessound clinical judgment based in best practice standards andevidence-based medicine Prescribes pharmaceuticals according topatient needs and conducts appropriate patient follow-up & outreachEnsures timely retrieval of laboratory and ancillary studies anddiagnostic consultation reports Consults with appropriate internaland external staff to make appropriate referrals for acute, chronicand preventive care Coordinates patient care and visits withappropriate clinical and non-clinical staff Documents patienthealth care files and completes appropriate forms according to HHCMG policies and procedures, including data collection andassessment plan based on patient needs and health care statusCarries out clinical interventions according to established plan ofcare and adapts actions based on changes in patients’ needsCounsels’ patients and their families with concern and empathywhile respecting confidentiality and privacy Initiates call-backswithin 8 hours for urgent care; within 24 hours for triage messagesDemonstrates commitment to ongoing education and improvement asevidenced by documentation of attendance at conferences,in-services, seminars, and committee meeting Maintains activeinvolvement with HHC MG’s APP Council Contributes to thedevelopment of clinical policies, standardized procedures, andclinical pathways Performs other related duties in support ofefficient department operations and superior patient care Shift -32 – 40 hours weekly Qualifications Eligibility to be licensed inConnecticut as a Physician Assistant (PA), or Advanced PracticeRegistered Nurse (APRN), is required. Eligibility to be certifiedas a Physician Assistant (PA), or National Board Certification as aFamily Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), orAdult Gerontology Primary Care Nurse Practitioner (AGPCNP) isrequired. Current State of

CT PA/APRN

license with prescriptiveauthority and current DEA number & 100% attendance to maintainannual mandatory skill updates 2 years of experience in generalsurgery/colorectal surgery, ambulatory surgery or acute caresetting for surgical care preferred Excellent clinical managementskills and proven ability to work collaboratively in a teamenvironment and take lead when appropriate Positive,customer-focused approach, with a commitment to providing excellentpatient care Excellent written and verbal communication skills.

Ability to communicate in other languages highly desirable Abilityto communicate technical information clearly to patients of allages Highly organized with demonstrated ability to effectivelyprioritize competing tasks with full autonomy Knowledge of computersystems, including experience with medical software (IDX preferred)Ability to adhere to HHC MG policies and standards of service aswell as HIPPA requirements, OSHA regulations and Infection Controlstandards Communicates tactfully and effectively and demonstrates acommitment to superior patient care Establishes and maintainseffective work relationships and an achievement-oriented attitudeDisplays courtesy and helpfulness when interacting with allinternal and external customers Exhibits strong personal initiativeand proactively acts to achieve goals beyond what is required orassigned Supports new ideas and exercises flexibility as changesare initiated while promoting a team atmosphere of trust andprofessionalism Completes requisite Risk Management trainingactivities as required
