• Perform duties including speaking with patients abouthealthy lifestyle choices, completing diagnostic tests to helpdiagnose and treat patients for injuries or illnesses andcoordinating with other healthcare professionals to maintain acomprehensive plan of care
• Encourage patients to become activeparticipants in their own healthcare and well-being throughservices such as health screenings, behavioral health and wellness,chronic condition management

Required Qualifications:

• A minimumof one year experience as a Nurse Practitioner is required.

Experience in Family medicine or primary care practicepreferred

• Active Family Nurse practitioner license in goodstanding
• Provide holistic, evidenced-based care inclusive ofaccurate assessment, diagnosis, treatment, management of healthproblems, health counseling, and disposition planning for ourpatients. This includes education and treatment for pregnancyprevention, sexually transmitted infection (STI) Prevention andsafer sex practices
• Order and administer immunizations andinjections (to include but not limited to COVID19, influenza, TB,Depo) and provides evidence-based patient/parent vaccine counselingand education
• Minimum of two years of medically relevantexperience or equivalent
• Complete the DOT certification examwithin 30 days of employment
• Basic Life Support (BLS)certification, including but not limited to performing CPR andoperating an Automated External Defibrillator (AED)
• Pass a FITtest and wear the provided respirator mask when protocolrequires.

Education:

• Master’s Degree level Family NursePractitioner program with current National Board Certification andState of Employment license to practice in the Advanced PracticeNurse rolePreferred Qualifications
• EPIC experience preferredPayRangeThe typical pay range for this role is:$87,035.00
• $187,460.00This pay range represents the base hourly rate or baseannual full-time salary for all positions in the job grade withinwhich this position falls.
• The actual base salary offer willdepend on a variety of factors including experience, education,geography and other relevant factors.
• This position is eligiblefor a CVS Health bonus, commission or short-term incentive programin addition to the base pay range listed above.
