Job Description
Trinity Health Of New England seeks a Nurse Practitioner or Physician Assistant to join the Cardiology Heart Failure team at Saint Francis Hospital and Medical Center.
RESPONSIBILITIES
Seeing patients admitted with heart failure in consultation with the cardiology team Performing advanced heart failure consultations Working with the interdisciplinary team made up of hospitalists, social workers, nurses, Cardiologists Working as an extension of the physician in managing patients with heart failure.
Skillset includes:
Management of diuretics, Guideline directed medical therapy initiation, Management of cardiogenic shock Attending interdisciplinary rounds daily The successful candidate will: Be interested in heart failure care Have Cardiology/Heart Failure experience (preferred but not required) Hold an Advanced Practice Nurse or Physician Assistant CT state license.
