Growing together. Primary Care Plus is looking for well qualified Internal Medicineand Family Practice physicians to join our growing practice in NEWORLEANS, Louisiana. PCPlus clinics are part of a clinicallyintegrated network designed to provide enhanced care to MedicareAdvantage, Quality plan, and ACO members by delivering highquality, coordinated healthcare that produces superior outcomes ina cost effective manner. Our clinical team includes physicians,APRNs, RN Care Managers, clinic support staff, and a QualityImprovement team who work together to coordinate care for patientsin both the outpatient and inpatient settings. A robusttransitional care and high risk outreach program providesphysicians with additional information to support their patientsoutside of the clinic. PCPlus also uses a sophisticated electronic medical record systemand robust analytical data analysis to identify patients who arehigh risk for poor outcomes in order to improve their quality oflife. Consistent feedback is provided to the clinical team on allbusiness objectives including quality measures, diagnosticaccuracy, and hospitalization rates. Physicians are not measuresbased on productivity or RVUs, but rather on outcomes of theirpatient panels. Most physicians will care for panels in the 600-800range and appointments are scheduled for 30-60 minutes perpatient. The Primary Care Physician (PCP) is a Licensed/BoardCertified/Board Eligible trained professional in internal or familymedicine who plays a key role as part of the clinical operationsteam providing direct patient care and providing assessmentsprimarily in the ambulatory health care center setting or elsewhereas deemed necessary. It also includes the participation in clinicalrounds and conferences plus in-depth documentation through writtenprogress notes and summaries. The PCP will be required todemonstrate the ability to function both independently and incollaboration with other health care professionals. The PCP willconsult with the applicable managers and medical directors toensure compliance with guidelines along with participating in riskand quality management programs, clinical meetings and othermeetings as required. The PCP will adhere to strict departmentalgoals/objectives, standards of performance, regulatory compliance,quality patient care compliance, and policies and procedures. Primary Responsibilities:

Assisting in the development of the plan of care in addition toproviding appropriate patient/ family/significant other counselingand education Ordering and interpreting appropriate laboratory and diagnosticstudies Ordering of appropriate medication and treatments Referring patients for consultation when indicated i.e.dermatology, neurology, ophthalmology, endocrine, surgery,intensive care, infectious disease, hematology, psychiatry, socialservice, dietary, etc. Documentation through in-depth progress notes and summaries Participates in patient care rounds and conferences Communicates patient management strategies to members of thepatient care team Collaborates with members of the multidisciplinary team to ensurethat patient management strategies are successful in meetingpatient care needs Recognizes situations which require the immediate attention andinitiates life-saving procedures when necessary

Required Qualifications:

Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), orM.B.B.S Board certification or eligibility in Family or InternalMedicine Active, unrestricted medical license in Louisiana Valid DEA license

Preferred Qualifications:

Certification in Basic Cardiac Life Support Board certification in Internal Medicine or Family Medicine 1+ years of clinical experience in geriatric, adult or familypractice setting The salary range for this role is $226,000 to $366,000 annuallybased on full-time employment. Salary Range is defined as totalcash compensation at target. The actual range and pay mix of baseand bonus is variable based upon experience and metric achievement.

Pay is based on several factors including but not limited to locallabor markets, education, work experience, certifications, etc.
