Primary Care Plus Physician Job DescriptionPrimary Care Plus is seeking qualified Internal Medicine and Family Practice physicians to join our expanding practice in NEW ORLEANS, Louisiana.
Our PCPlus clinics are part of a clinically integrated network aimed at providing enhanced care to Medicare Advantage, Quality plan, and ACO members through high-quality, coordinated healthcare that achieves superior outcomes cost-effectively.
The clinical team includes physicians, APRNs, RN Care Managers, support staff, and a Quality Improvement team working together to coordinate outpatient and inpatient care.
A transitional care and high-risk outreach program offers additional support to physicians and patients outside the clinic setting.
We utilize advanced electronic medical records and analytical data to identify high-risk patients and improve their quality of life.
Feedback on quality measures, diagnostic accuracy, and hospitalization rates is regularly provided to the clinical team.
Physicians are evaluated based on patient outcomes rather than productivity or RVUs.
Typical panels range from 600-800 patients, with appointment times of 30-60 minutes per patient.
The Primary Care Physician (PCP) is a licensed, board-certified or eligible professional in internal or family medicine, providing direct patient care primarily in the ambulatory setting.
Responsibilities include clinical rounds, documentation, and participation in conferences.
The PCP must operate independently and collaboratively, complying with guidelines, participating in quality programs, and adhering to departmental standards, policies, and regulations.

Primary ResponsibilitiesDevelop and implement care plans, providing counseling and education to patients and families.
Order and interpret laboratory and diagnostic tests.
Prescribe medications and treatments appropriately.
Refer patients to specialists such as dermatology, neurology, ophthalmology, etc., as indicated.
Document thoroughly through progress notes and summaries.
Participate in patient care rounds and team conferences.
Communicate management strategies to the healthcare team.
Collaborate with multidisciplinary team members to ensure effective patient care.
