PrimaryResponsibilities:

• Direct and manage a complete continuum ofhigh-quality care within the care team
• Treat chronic diseases anddisorders
• Reviews patient file/record, including allergies,problems, medications, and immunization status
• Elicits andrecords information about patient’s medical history
• Orders orexecutes various tests, analyses, and diagnostic images to provideinformation on patient’s condition

Required Qualifications:

• Unrestricted New York State MedicalLicense or ability to obtain prior to employment
• BoardCertification or Board Eligibility in Family or Internal Medicine
• Active and unrestricted DEA license or ability to obtain prior toemployment The salary range for New York residents is $249,728 to$355,092 per year.

Salary Range is defined as total cashcompensation at target. The actual range and pay mix of base andbonus is variable based upon experience and metric achievement.
