Great Life work Balance position with excellent benefits! We areseeking a Physician who is passionate about providing quality careto the geriatric population. The Physician participates as a memberof the PACE Interdisciplinary Team in the care of high risk olderadults living in the community with a strong focus on theparticipant’s goals as well as excellent geriatric care management.

The Physician is the leader of the Clinical Team whose primarymission is to assist frail older adults to remain living in thecommunity for as long as it is safe. Care meets the objectives,standards and policies of the PACE model. Incumbent demonstratesproficiency in collaborating with an interdisciplinary team andtreating participants with multiple chronic and acute illnessesfrom diverse backgrounds. This is a full-time position, Monday-Friday 8-4 position; no nights, weekends or holidays!

Responsibilities:
  • Completes initial, periodic and annual physicalexaminations of the participants.
  • Evaluates participants forepisodic visits, for acute illnesses and for follow-ups ofhospitalizations in the clinic, at home or at a nursing home.
  • Makes nursing home visits in accordance with the state nursing homerequirements and as needed.
  • Use telehealth as needed and whereappropriate
  • Participates in the PACE hospital rounding system andthe on-call schedule.
  • Evaluates the needs of the participants anddevelops treatment plans in concert with the other members of theInterdisciplinary Team.
  • Perform other duties as required.
Qualifications:
  • Bilingual/Bicultural desirable.
  • Demonstratedability to work within urban communities, particularly withracially, ethnically, and culturally diverse, low income, andspecial needs populations.
  • Board Certified in Internal Medicineor Family Practice, or eligibility for Board Certification (forrecent graduates).
  • Board Certified in Geriatric Medicine,preferred.
  • Current License to practice medicine in theCommonwealth of Massachusetts.
  • Current Massachusetts and DEAPrescription Privileges. Suboxone Waiver preferred.
  • Maintainsappropriate hospital and long term care facility credentials andprivileges.
  • Current CPR Certification.
  • 1 year geriatricsexperience preferred
  • Ability to be able to relate well toparticipants, to anticipate their needs and to encourage theirindependence.
  • Preferred:

    At least one (1) year of experienceworking with low income and diverse cultural groups.

  • Ability toconvey information clearly and succinctly with team members.
  • Ability to be able to work well with others and to be a dedicatedteam member.
  • Ability to pay attention to detail, to be aself-starter, and to complete assignments on time with minimaldirection.
  • Ability to be able to quickly recognizesituations/problems before they become acute, and to be able toimplement effective solutions and understand consequences.
  • Mustbe comfortable with palliative and end of life care.
  • Must becomfortable doing home care visits
  • Proof of Covid vaccinerequired.
