The Primary Care Physician (PCP) works as alead in our team-based care environment. We are a value-based careprovider focused on quality of care for the patients we serve. Ourcare team consists of Doctors, Advanced Practice professionals,Pharm D, Care Coach Nurses, Medical Assistants, Behavioral Health,Specialists, Quality Based Coders, Referral Coordinators and more.
Our approach allows us to provide an unmatched experience forseniors. Our model is positioned to provide higher quality care andbetter outcomes for seniors by providing a concierge experience,multidisciplinary services, coordinated care supported by analyticsand tools, and deep community relationships. This robust supportallows our PCP to see fewer patients and spend more time with thosethey do. Total compensation package (base pay bonus) could exceed$300K, depending on experience and location . The Physician focuseson outpatient medicine, continuity of care, health maintenance, anddisease prevention. Keeps a medical history and medical records.
Refers the patient to specialists as needed. Begins to influencedepartment’s strategy. Makes decisions on moderately complex tocomplex issues regarding technical approach for project components,and work is performed without direction. Exercises considerablelatitude in determining objectives and approaches to assignments.

Required Qualifications:
Currentand unrestricted medical license or willing to obtain a medicallicenses in state of practice; eligible and willing to obtainlicenses in other states in the region of assignment, as requiredGraduate of accredited MD or DO program of accredited universityBoard Certification or Board Eligible in Family Medicine orInternal Medicine (Geriatric Medicine preferred) Active andunrestricted DEA license Excellent verbal and written communicationskills Demonstrate a high level of skill with interpersonalrelationships and communications with colleagues/patients Fullyengaged in the concept of “Integrated team based care” modelWillingness and ability to learn/adapt to practice in a value-basedcare setting Superior patient/customer service Basic computerskills, including email and EMR This role is considered patientfacing. Minimum of two to fiveyears directly applicable experience preferred Experience managingMedicare Advantage panel of patients with understanding of BestPractice in coordinated care environment in a value-basedrelationship environment Knowledge of Medicare guidelines andcoverage Knowledge of HEDIS quality indicators
