Job Description Current Department Staffing:
Primary Care Service Chief; 2 Lead Physicians; 24.0 FTE PACT PCP’s and 3.0 FTE GAP Physicians; 2 GAP NP’s; 3 Admin Staff. This position will report to the Section Chief of Primary Care.
Practice Environment Description:
Panel size averages 1100. This is a non-supervisory position.
Highlights of this position include:
Daily schedule: Monday•Friday, 8:00am•4:30pm. Physician will be assigned to a Patient Aligned Care Team (PACT). The PACT care model consists of the following: PCP, RN, LPN, and Clerk.
Additional ancillary staff:
Social Work, Pharmacy, Mental Health, and Physical Therapy are located within the clinic.
No Call Coverage Additional requirements:
Board Certified or Board Eligible. Family Practice or Internal Medicine Licensure.
