Position SummaryThe Primary Care Physician (MD/DO) diagnoses andtreats diseases and injuries of human internal organ systems anddelivers preventive and therapeutic treatment of primary careneeds, including chronic disease management, of patients throughroutine physical examination testing and non- invasivetreatment.

Core Responsibilities Patient Care:

Actively diagnoses and treats our veterans. Examines patient for symptoms of organic or congenitaldisorders and determines nature and extent of injury or disorder,performs comprehensive physical examination, referring todiagnostic mages and tests, and using medical instruments andequipment and documents all medical information into the patient selectronic medical record utilizing the VA s

electronicpatient record system. Maintains confidentiality of veterans dataand information. Develops and implements patient management plans and assists inprovision of continuity of care. Orders and performs diagnostic tests, such as x-ray,electrocardiogram, laboratory tests, etc. and interprets testresults for deviations from normal and provides patientnotification and follow up care. Prescribes medication and recommends dietary and activityprograms as indicated by diagnosis. Refers patient to the VA medical specialist whenindicated. Counsels patients on the use of prescription medications,educates patients, assesses mental health issues, and providesroutine health maintenance. Evaluates patients records from medical providers outside theVA and works with these patients utilizing rules set forth by theVA for co-managed care. Completes any and all clinical reminders due at the time ofeach patient visit. Completes the documentation of the medical record withintwenty-four 24 hours of a patient encounter. Solicits medical management consult or input whenappropriate. Will comply with all VA guidelines in regard to appropriate andtimely response to all patient request, alerts and notifications,consults, orders, lab results and diagnostic studies. Complies with the VA formulary process and consultprotocols. Serves as collaborative physician to mid-levels whereappropriate and needed. Complies with all VA guidelines in regard to appropriate andtimely clinical documentation, including the completion of allencounters by close of business. Addresses the clinical needs of all walk-in/triage/urgent carepatients. Agrees to cross cover other providers, including alerts andnotifications. Complies with all VA and company training requirements. Qualifications Doctor of Medicine, Doctor of Osteopathic Medicine orequivalent from a school in the United States or Canada Completion of an accredited residency program in familymedicine, or internal medicine. 5 years of chronic disease management experience, afterresidency training Current, active, full and unrestricted license or registrationas a physician in ANY state. Board-certified in Internal Medicine, Family Medicine, orPreventive Medicine Must comply with and maintain all requirements for a valid,unrestricted license in the state of desired employment, or in anyU.S. state or territory, depending on VA contractualrequirements. Current certification in Basic Life Support, must be renewedperiodically as specified by the AHA, and in ACLS if specified byindividual VAMC contract. Current, unrestricted Drug Enforcement Administration (DEA)registration Annual PPD test Must be compliant with federal, state, local government orcontract specific vaccine mandates and/or requirements.
