Videos Duties The Primary Care/PACT-Pain clinician delivers evidence-based comprehensive management through direct pient care and as part of the interdisciplinary PACT/Primary Care team providing care of acute and all chronic medical problems and Pain Management. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective trements; helping achieve positive pient centric outcomes (e.g. establishing and progressing towards individualized function-reled goals, improving quality of life, minimizing theoretical risk of drug-reled harm) through direct and indirect interactions with pients, sub-specialities, and interdisciplinary team in Primary Care; performing physical examinions; implementing risk mitigion stregies and ordering laborory and other tests to help determine efficacy and toxicity of medicion therapy. The PACT clinician manages initiing, medicion therapy by modifying and discontinuing medicion both independently and in collaborion with other practitioners when needed (e.g., buprenorphine prescribing for Opioid Use Disorder, controlled substances).
FUNCTIONS
Clinical –
Direct Pient Care Activities:
Maintains and promotes high ethical, clinical, and administrive standards th are carried into the decision-making process. Provides pient care which is approprie to the cognitive, emotional, and chronological murion needs of the adult and/or geriric pient. Incumbent must demonstre the knowledge and skills necessary to provide care approprie to the age of the pients served. Functions as a physician for Primary Care services to achieve defined outcomes through direct interactions with pients and other clinicians within PACT/Primary Care team the Kernersville HCC locion, which may include, but are not limited to: Provides primary care services as specified in his/her clinical privileges. Services include new pient evaluions and ongoing monitoring through follow up s. Procedures such as joint injections, btlefield acupuncture are preferred but not required. Prescribes, monitors, and adjusts medicions reled to chronic disease ste and pain management Clinical – Indirect Pient Care Activities Reviews pient medicion regimens for clinical effectiveness, drug selection, dosing, contraindicions, side effects, potential drug interactions, and therapeutic outcomes as required. Communices findings with prescribers and provides approprie recommendions for alternives to current trement plans as needed. Provides input and recommendions as the pain expert on the interdisciplinary pain management team during routine pient care planning. Documents clinical interventions and pient care encounters in the electronic health record in a timely and professional manner as approprie. Works with team members to ensure compliance with nional, VISN, and local initiives (e.g., OSI) Participes effectively in team meetings and trement planning conferences and collabores with multidisciplinary team members in a manner th enhances coordinion of comprehensive pient care. Provide recommendions and work with the interdisciplinary team to coordine care to provide optimal functioning, level of pain, and quality of life for Veterans. Be accessible and available to personnel in the PACT team for consultion on complex cases. Ensure compliance with all legal and regulory requirements. Ability to lead Process/Performance Improvement (PI), and quality assurance initiives for primary care and pain reled care delivery.
Research:
(when applicable) Participes in design and/or completion of research studies and/or quality improvement projects involving drug therapy, clinical pharmacy functions, or improvements in pient care. Directs and/or participes in the preparion of articles for publicion.
Program Management and Administrion:
Participes and facilites departmental and organizional compliance with The Joint Commission (TJC), Office of General Counsel (OIG), and other applicable regulory and accredition regulions and guidelines regarding pient safety and care. Supports efforts to improve performance metrics specific to pain management. Maintains effective interdepartmental relionships by committee membership, barrier resolution, active problem resolution skills, and close cooperion with other services in accomplishing the medical center mission and goals. Collabores with the Salisbury VA Facility Pain Management Coordinor (PMOP), Pain consultant (POC), Facility Pain Champions to maintain best pain practices Participes in various hospital committees, department and hospital in-service programs and acquire CMEs as required.

Basic Requirements:
United Stes Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of educion in medicine or osteophic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a Ste, Territory, or Commonwealth of the United Stes, or in the District of Columbia.
Residency Training:
Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certificion. (
NOTE:
VA physicians involved in academic training programs may be required to be board certified for faculty stus.) Approved residencies are: (1) Those approved by the Accredition Council for Gradue Medical Educion (ACGME), b) OR[(2) Those approved by the American Osteophic Associion (AOA),OR(3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United Stes), which the local Medical Staff Executive Committee deems to have provided the applicant with approprie professional training and believes has exposed the physician to an approprie range of pient care experiences. Residents currently enrolled in
ACGME/AOA
accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as “Physician Resident Providers” (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rher, they are to have a “scope of practice” th allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English.

Physical Requirements:
Must be able to physically and mentally perform the duties of Physician under normal and emergent conditions. Modere lifting 15-44 lbs and over with assistive devices, heavy carrying of 15-44 lbs and over with assistive devices; reaching above shoulder; use of fingers, both hands required, walking or standing up to 4 hours per day; repeed bending up to 1-2 hours per day; ability for rapid mental and muscular coordinion; near vision correctable; hearing aid permitted. For foreign medical gradues not covered in (1) or (2) above, facility officials must verify with the Educional Commission for Foreign Medical Gradues (ECFMG) th the applicant has met requirements for certificion, and must obtain a copy of the ECFMG certifice, if claimed by the applicant. Additional informion The PACT clinician will follow pients in direct care setting to achieve therapeutic goal and make referrals as needed to optimize pient outcomes and trement success. The PACT clinician refers to the approprie level of care for pients when needed and is responsible for providing and documenting care of assigned pients. The PACT clinician has knowledge of VA/DOD clinical practice guidelines, The Joint Commission (TJC) standards, Veterans Affairs (VA) performance measures, and will integre these standards into practice. The Pain PACT clinician may participe as a subject mter expert on pain-reled committees the local facility level (e.g., Pain Committee) and as permitted nional and VISN levels. The PACT clinician should assist with quality improvement projects reled to pain. Indirect pient care activities include participing in quality assurance and improvement projects, supporting research, staff educion and consultion.
