Seeking a physician board certified or eligible in InternalMedicine, Family Medicine, or Geriatric Medicine DEA – Required Candidates with 2+ years prior direct experience and knowledgeof HEDIS quality indicators are strongly encouraged to apply Bilingual is a plus 100% Outpatient Responsibilities include: Admin time Assists in the coordination of patient services, including butnot limited to specialty referrals, hospital and SNF coordination,durable medical equipment and home health care Meets with RMD about quality of care, review of outcome data,policy, procedure and records issues Participates in potential growth opportunities for new orexisting services within the Center Participates in the local primary care “on-call” program ofCenterWell as needed Assures personal compliance with licensing, certification, andaccrediting bodies Spend 100% of your time clinically focused on direct patientcare, inclusive of patient facing time and general administrativetime (charting, meetings, etc.) as it relates to direct patientcare

Schedule:

Monday – Friday Minimal call Geriatric/Medicare Advantage Patient Base This role is considered patient facing and is a part of ourTuberculosis (TB) screening program. If selected for this role, youwill be required to be screened for
