Advanced PracticeClinicians Primary Responsibilities:

• Direct and manage a completecontinuum of high-quality care within the care team
• Treat chronicdiseases and disorders
• Reviews patient file/record, includingallergies, problems, medications, and immunization status
• Elicitsand records information about patient’s medical history
• Orders orexecutes various tests, analyses, and diagnostic images to provideinformation on patient’s condition

Required Qualifications:

• Unrestricted licensure in the stateof New York or has New York application in process
• Residencytrained and Board Certification or Board Eligibility in FamilyMedicine and/or Internal Medicine
• Active and unrestricted DEALicense or ability to obtain prior to start The salary range forNew York residents is $256,997 to $364,843 per year.

Salary Rangeis defined as total cash compensation at target. The actual rangeand pay mix of base and bonus is variable based upon experience andmetric achievement.
