Our care teamconsists of doctors, advanced practice professionals, Pharm D, carecoach nurses, MAs, behavioral health specialists, quality basedcoders, referral coordinators and more. Our model ispositioned to provide higher quality care and better outcomes forseniors by providing a concierge experience, multidisciplinaryservices, coordinated care supported by analytics and tools, anddeep community relationships. This robust support allows our PCP tosee fewer patients and spend more time with those they do. $75,000Sign-on Bonus Total compensation package (base pay bonus) couldexceed $300K depending on experience and location.

Responsibilities:

Evaluates and treats center patients inaccordance with standards of care. Follows level of medical careand quality for patients and monitors care using a vailable dataand chart reviews. Assists in the coordination of patient services,including but not limited to specialty referrals, hospital and SNFcoordination, durable medical equipment and home health care. Actsas a n active parti cipant and key source of medical expertise withthe care team through daily huddles. Helps Regional MedicalDirector and Center Administrator in setting a tone of cooperationin practice by displaying a professional and approachable demeanor.

Completes all medical record documentation in a timely mannerworking with a quality- based coder to optimize coding specificity.

Follows policy and protocol defined by Clinical Leadership. Meetswith RMD about quality of care, review of outcome data, policy, procedure and records issues. Participates in potential growthopportunities for new or existing services within the Center.

Participates in the local primary care “on-call” program ofCenterWell as needed. Assures personal compliance with licensing,certification, and accrediting bodies. Spend 100% of your timeclinically focused on direct patient care, inclusive of patientfacing time and general administrative time (charting, meetings,etc.) as it relates to direct patient care. Required Qualifications:

Current and unrestricted medicallicense or willing to obtain a medical licenses in state ofpractice; eligible and willing to obtain licenses in other statesin the re gion of assignment, as required Graduate of accredited MDor DO program of accredited university Excellent verbal and writtencommunication skills Demonstrate a high level of skill withinterpersonal relationships and communications withcolleagues/patients F ully engaged in the concept of “Integratedteam based care” model Willingness and ability to learn/adapt topractice in a value based care setting Superior patient/customerservice Basic computer skills, including email and EMR This role isconsidered patient facing and is a part of CenterWell’sTuberculosis (TB) screening program. If selected for this role, youwill be required to be screened for
