Same Day Clinic Provider- MD/DO/APP Townshend, VT
Description Grace Cottage Family Health is seeking a Same Day Provider to assess, diagnose, and treat patients of all ages with urgent medical needs. Responsibilities include performing exams, ordering and interpreting diagnostic tests, prescribing treatments and medications, and providing health education. This role supports transitional care management, ED follow-ups, and sports physicals as needed, and works closely with the care team to ensure high-quality, timely patient care. Life And Disability Insurance Key Responsibilities of the position: Assess, diagnose and treat patients of all ages with illnesses or injuries that require immediate attention. Interview patients concerning their symptoms and conduct a thorough physical examination.
Order or execute various tests, analyses, and diagnostic images to provide information on patients’ conditions.
Interpret reports and findings of tests and examinations and diagnose conditions of patients.
When clinically indicated, administer or prescribe treatments
Determine and prescribe medication, dosage, and schedule given the patients’ conditions and allergies.
Discuss any possible side effects to medication or immunization with patients. Utilize online resources to perform travel consultations.
Promote health by advising patients about diet, hygiene, and methods for prevention of disease.
Refer patients to primary care physicians or medical specialists for consultation services when necessary for the well-being of the patients.
Document the patients’ visits including medical history, physical exam, diagnoses, and plan of action.
Follow-up with patients regarding progress in high risk or emergency cases. Follow-up daily on laboratory tests and X-Rays.
Assist GCFH clinic with transitional care management, ED follow up, and school and sports physical visits, when needed.
Consults with the Medical Director and other professionals on staff as needed regarding patient care, assessment, and education issues.
Uses the electronic health record to document patient care and completes records according to Grace Cottage policy.
Maintains confidentiality of patient information according GC policy and federal/state guidelines and regulations.
May serve as a preceptor to medical students or other appropriate students per federal and state regulations.
Adheres to Grace Cottage Medical Staff Bylaws, Rules and Regulations, Standards of Behavior, policies and procedures, and Granted Privileges.
Participates in quality improvement, management, continuing education, and other patient care programs established by Grace Cottage.
Assists in the resolution of complaints, requests and inquiries from patients
Requirements Qualifications:
Licensed or willing to license to practice in Vermont.
Graduate from accredited Medical College w/medical degree & completion of a US residency program.
Have a record that is free from current Medicare/Medicaid sanctions and not be listed on either the OIG List of Excluded Individuals/Entities or System for Awards Management (SAM).
Possess a current, valid, unrestricted Drug Enforcement Administration (DEA) number if applicable.
Board Certified per Grace Cottage Bylaws.

$125k/yr- $235k/yr based on experience/licensure
