JOB SUMMARY
The duties of a LMHCor LCSW include, but are not limited to, commute to multiplepost-acute care facilities every day, making rounds, communicatingto patients, facility staff members, providing accurate assessment,diagnosis, treatment, consultation, education and follow-up care topatients. Work with Psych division providers within the post-acutefacilities.
MUST HAVE A FLORIDA LICENSE OR BE A REGISTERED INTERNIN FLORIDA.
Monday-Friday 15-20 Patients per day 1099/W2 Flexiblehours
PRIMARY FUNCTIONS 1.
Examine patients and record history 2.Adhere to proper charting protocols 3. Discuss with Physician andother health care professionals to prepare comprehensive plan 4.Submit health care plan and goals of individual patients forperiodic review by Physician 5. Promote health by advising patienton diet, hygiene and prevention of disease 6. Refer to specialistas required for patient’s well-being 7. Report deaths andcontagious diseases to governmental authorities 8. Otherresponsibilities as assigned
EDUCATION AND EXPERIENCE 1.
Must haveand maintain current State of Florida license in good standing 2.Must be
LMHC/LSW
in the State of Florida 3. Current National BoardCertification
KNOWLEDGE, SKILLS, AND ABILITIES 1.
Effectivewritten, verbal and electronic communication skills 2. Showinitiative, creativity and problem-solving ability 3. Ability tocollaborate with colleagues to provide quality healthcare
ADDITIONAL QUALIFICATIONS 1.
Prior experience using an electronicmedical records system 2. Bilingual a plus, but not required(English/Spanish)
