We exist to enhance the lives of our residents, and we are actively seeking an experienced Discharge Planner to join our family! This is a Full-time position. Monday – Friday 8:30am to 5pm (On-call every 12 weeks for Saturday and Sunday) Responsibilities include, but are not limited to: Provide guidance and counsel to residents and families regarding post discharge care plans, placement options and equipment needs Facilitate the discharge planning process through coordination with the interdisciplinary team and serves as a liaison to safely transition residents to the appropriate level of care Benefits include: Excellent pay with BCBS Health & Dental Insurance Generous number of PDO’s FREE Healthcare after 2 years of employment Paid Nurse Scholarship after 6 months of successful employment Come grow with us!
