Position Responsibilities:
  • Conduct an in person initial comprehensive recreational assessment. Collaborate with the interdisciplinary team to develop a comprehensive care plan for each participant
  • Conduct an in-person reassessment semiannually and as needed
  • Maintain regular attendance at, and participate in, interdisciplinary team meetings communicating participant changes, collaborating on care planning decisions and coordinating 24 hour care delivery
  • Plan and implement group and individual activities to improve or maintain the participants’ cognitive, physical, social, psychosocial and artistic needs
  • Plan and implement activities for dementia-specific population. Oversee daily flow of dementia activities area. Activities may focus on small groups, one-on-one programming and sensory stimulation.
  • Organize community outings and special events at the Eldercare Center.
  • Provides recreational educational and training to support Recreation and Activities Assistants (CNAs) and other Center staff members as appropriate
  • Creative ability to plan, develop and implement varied activities to meet diversified needs.
  • Maintain participants’ medical records and fulfill Eldercare charting and reporting requirements
  • Provide leisure education to participants and caregivers for activity modification, use of adaptive equipment, activity skill development and recommended supplies for in-home use.
  • Maintain general order in the activities area, including daily clean-up, routine supply inventory ordering, petty cash and overall general budgeting.
  • Develop and post a monthly calendar of activities.
  • Oversee the aides working in the care center including activities, dietary, clinic and rehab activities, including hiring, counseling and firing certified nurse aides to work in the center
  • Increase participant’s involvement in the community by organizing at least one special event a year.
  • Participate in and support Quality Improvement initiatives
  • Act only within the scope of his or her authority to practice
  • Know and adhere to philosophy and goals of
Eldercare Qualifications:
  • Graduate of an accredited Recreational Therapy Program or, Certification as an Activity Director. BLS or CPR Required.
  • Minimum of 2 years working with a frail or elderly population
