This position assists all departments in supporting the Optimal Living® philosophy and the seven dimensions of wellness (physical, emotional, spiritual, intellectual, environmental and occupational). Resident Relations assists in the gathering of and dissemination of information in support of resident admissions, orientation, continuing stay and resident move outs. This position monitors and creatively seeks ways to prevent “back door” loss of residents and communicates to management the early identification of possible resident move-outs. ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Other duties as assigned. Prepares and details aspects of resident move in, and coordinates move with Marketing Director, Housekeeping Manager, Maintenance Manager, and Reception Coordinator to insure all the details of move in are implemented. Maintains orientation notebooks and provides move in gift. Conducts facility orientation for new residents and their families as appropriate, advises on site set-up for telephone, additional cable television services, newspapers, etc., by providing residents with the contact information. Works closely with residents, families, Enrichment Director and Wellness Director to explore resident interests and facilitate the engagement of the resident’s interests. Serves as a liaison between resident/family and community provided services such as Outpatient Therapy, Supported Healthcare Services and Physician Services. Partners with Vitality Care, Therapy Services and Healthcare Support services to lead Interdisciplinary Team in weekly meetings to communicate the needs of residents and to arrange for support. Leads the community’s Ambassadors program, where current residents help support new residents during their initial transition period into the community. Performs wellness checks and alerts healthcare providers as needed. Addresses conflicts that may arise to find common ground and settle disputes equitably using grievance form and proper follow up procedures. With the Executive Director’s guidance, facilitates discussion with families when a resident has health concerns and advocates for the resident’s needs. When a resident is no longer independent, guides the family to find more appropriate living arrangements. Apprises other Department Directors with resident care concerns, health changes and needs of residents. Arranges that cable services are set up prior to move in. Troubleshoots issues as appropriate for issues with cable and Wi-Fi connections, arranging for outside support. Conducts weekly visits of residents in the hospital and rehab.
ENTRY QUALIFICATIONS
Bachelor’s degree required Social Work background/area of study preferred Proficient in the use of Microsoft Word required Experience using Docusign and Canva preferred
SUPERVISORY RESPONSIBILITIES
Supervision of one departmental employee and as needed, volunteers for assigned activities.
