Care plans Full Job Description Brooklyn Gardens, a vibrant and resident-centered skilled nursing facility in Brooklyn, NY, is seeking a creative and organized Activities Director with prior experience in a nursing home or SNF setting. We’re looking for someone who can bring energy and innovation to our daily recreational programming—enhancing quality of life and engagement for every resident.

Responsibilities:

Plan, organize, and lead a variety of activities including arts & crafts, music, games, and social events Develop monthly activity calendars tailored to residents’ interests and needs Engage residents of all cognitive and physical levels, including one-on-one and group sessions Maintain documentation, attendance records, and care plan input in compliance with regulations Collaborate with nursing, rehab, and social services teams to support holistic resident care Coordinate special outings and events with families and the community

Requirements:

Prior experience as an Activities Director or in a similar role within a skilled nursing facility Strong organizational and interpersonal skills Knowledge of MDS, care planning, and recreational therapy principles Certified Therapeutic Recreation Specialist (CTRS) or Activity Director certification preferred Compassionate, creative, and resident-focused approach
