Resident Services Coordinator The Cottages at Garden Grove Cicero, NY 13039 The Cottages at Garden Grove “A place we all call home!” Looking for something different? Are you ready to love coming to work? The Cottages at Garden Grove is looking for motivated Resident Services Coordinator to join a great team in our person centered home environment. The Cottages at Garden Grove is set on a lovely campus in Cicero, NY and is made up of 12 small-home residences. This model of small-homes sets a new standard for skilled nursing and short-term rehab in CNY. These homes come complete with a sense of neighborhood and welcoming atmosphere that will make everyone who resides and works here feel at home.

Job Duties:

• Completes all required documentation appropriately
• Attends weekly rehab rounds meeting, and maintains, updates and distributes log
• Creates and issues Medicare/insurance cut letters by direction of IDCP Team, within appropriate regulatory guidelines
• Acts as liaison to the family and resident upon admission, including greeting the incoming admissions, providing information on contact lists and ensuring the admission pathway is complete
• Assist in the evaluation of Customer Satisfaction of Resident and Family surveys
• Schedules and coordinates Homecare referrals
• Coordinates hairdresser schedule for residents
• Enters MDS’s information into the system per social works assessment
• Assists with appointments, PPW and other related tasks for Residents discharging
• Orders necessary DME for Residents
• Schedules care plan meetings
