Located in the Midlands of South Carolina near the shores of beautiful Lake Murray, the Heritage at Lowman is a growing, robust community of active senior adults. The Active Life and Wellness Manager will plan, organize, develop and direct the overall operation of clinical care for the Active Lifestyle community and Independent Apartment residents. This position provides case management for Active Lifestyle and Independent residents. Acts in the absence of the Director of Active Lifestyle in conjunction with the Wellness & Fitness Manager.

Responsibilities:

Directs and supervises general case management, intake and referral services to all Active Lifestyle and Independent residents needing such assistance. Establishes linkages with all agencies and service providers in the community; determines and develops relationships with best service pricing, to assume/re individualized, flexible and creative service for the involved resident (S). Creates and maintains directory of providers that are available to both staff and residents. Refers and links residents to service providers in the general community. These are, for example, personal assistants, homemaking services, counseling, home health nursing, preventative health screening and legal advocacy. Utilizes BeWell Homes Services where appropriate. Coordinate with Life Enrichment to provide educational seminars/speakers regarding health and Wellness on subjects that are requested by or would interest the independent and Active Lifestyle and Independent residents. Monitors the provisions of services from community agencies and keeps the Dir. Of Active Lifestyle and the Leadership, team current on the progress of the individual resident(s). Responds to emergency calls in the building and independent areas of the campus and assists other staff as needed in responding to emergency calls. Professional development, attending workshops as needed to continue professional growth and maintain relationships in the community. Supervise the campus First Responders Manages the transportation department Provide Staff Development for clinical departments

QUALIFICATIONS

Bachelor’s Degree in Social work, Psychology, Gerontology, Human Services, Nursing or Occupational Therapy Must have the ability to make sound independent decisions Must be able to take evening and weekend call on a rotating basis Must be able to take emergency call Maintain a current CPR certification Detailed oriented and strong problem solving skills Excellent verbal and written communication skills Valid driver’s license Nursing degree preferred Come join our team and find personal and professional satisfaction as you share your talents and skills with us.
