Position Overview:

The Hospice Registered Nurse is responsiblefor pain management, symptom control in the delivery of care tohospice patients, as established by the plan of care, theinterdisciplinary team, attending physician and Hospice MedicalDirector.

Schedule:

Full-Time visits Monday – Friday with Fridaythrough Sunday on-call rotation

Baldwin CountyEssential Job Functions:

Ensures the timely and adequate deliveryof hospice services to the terminally ill patient and their family,operating within the plan of care as established by the hospiceteam and attending physician.

  • Assesses the total needs of thepatient/family during regularly scheduled and after hour homevisits;
Documents:

assessment, identified problems, nursinginterventions, goals, and outcomes of interventions.

  • Coordinatestotal patient/family hospice care under the supervision of theDirector and with the interdisciplinary team, the attendingphysician, and other providers; documents such coordination.
  • Collaborates with the interdisciplinary team in the development,review and revision of the clinical component of patient/familyplan of care, including exchange of information, review ofproblems, assessing effectiveness of interventions and documentingoutcomes.
  • Ensures continuity of care between patient/family, teammembers, ancillary providers, long term and inpatient carefacilities, and the attending physician.
  • Communicatespatient/family/caregiver needs, ongoing nursing assessment,interventions, goals and outcomes through the interdisciplinaryprocess.
  • Provides and documents continuing education of thehospice concept of care to patients and their families/caregivers.
  • Supervises the delivery of patient care provided by HospiceAides, Licensed Practical Nurses; ensuring compliance with theestablished plan of care and completing required documentation ofsupervision.
  • Facilitates the delivery of hospice services topatients residing in long term care facilities, documenting jointcoordination of care with facility professional staff, andattending facility care plan meetings as appropriate.
  • Participates in on-call rotation for delivery of care after officehours, on weekends, and holidays.
  • Provides ongoing monitoring ofpatient appropriateness for hospice services and completes requireddocumentation for certification and recertification

An active RN License in the state ofapplication

Valid CPR Preferred:

Medicare Skilled Nursingexperience 1-year RN experience in a hospice setting HHH As anemployer accepting Medicare and Medicaid funds, employees mustcomply with all health-related requirements in all relevantjurisdictions, including required vaccinations and testing, subjectto exemptions for medical or religious reasons as appropriate.
