Salary Range:

$38 – $51 Department Details 16-24 hours per week, Primarily dayshift hours,. Includes on-call rotation. Additional weekendincentive of $1.25/hour. Additional $1.25/hour shift incentive forany hours worked during the hours of 6pm-6am Job Summary Integrateshealth care for residents from pre-admission to post discharge.

This is accomplished through the coordination and sequencing of theresident’s care. Integration enhances resident flow andinterdisciplinary communication promotes early intervention. Seeksto evaluate outcomes based on an integration among establishedclinical, financial and utilization data. Functions in structuredand unstructured health care settings described as a geographicand/or situational environment that may not have establishedpolicies and procedures. Utilizes independent nursing judgment whenintegrating health care. Has knowledge of and utilizes appropriateage-specific structured care methodologies, such asprotocols/integrated clinical pathways/guidelines/standards of carerelating to the overall health care needs of the residents.

Interviews potential residents and assesses residents in compliancewith regulations, policy and procedure. Uses assessment informationfor admission and continued stay in the assisted living community.

Ensures all residents meet assisted living qualifications asidentified in the occupancy agreement. Establishes a service planfor each resident and evaluates on an on-going basis. Able to workwith growth and development needs of client populations in clinicalarea. Ability to communicate with residents, family members andothers on the health care team. Coordinates, facilitates, andnegotiates with others. Monitors, evaluates and trendspatient/residents responses utilizing structured caremethodologies. Fiscally responsible. Controls variances andduplication. Must demonstrate independence, assertiveness andcritical thinking when working with patients and co-workers.

Ability to document clearly. Ability to solve problems. Selfmotivated. Works with little direction. Ability to incorporateteaching/learning principles and adapt teaching to age group.

Ability to facilitate groups and implement projects when necessary.

Must be able to establish priorities, have strong flexibility andorganizational skills. Must be knowledgeable about reimbursementfor services provided. Will be required to work primarily dayhours, scheduled weekends, and occasional evenings. May be exposedto communicable or infectious disease, hazardous materials andinjury from performance of assigned duties. Is subject to multiplesensory and environmental stressors. Qualifications Bachelor’sdegree in Nursing strongly preferred. Graduate from an accreditednursing program, including, but not limited to, those accredited bythe Commission on Collegiate Nursing Education (CCNE),Accreditation Commission for Education in Nursing (ACEN), orNational League for Nursing Commission for Nursing EducationAccreditation (NLN CNEA). Minimum of two years’ clinical experiencepreferred. Currently holds an unencumbered RN license with theState Board of Nursing where the practice of nursing is occurringand/or possess multistate licensure if in a Nurse Licensure Compact(NLC) state. Obtains and subsequently maintains required departmentspecific competencies and certifications.
