Job Summary Responsible for directing and coordinatingall nursing care based on established clinical nursing practicestandards Collaborates with other professional disciplinesthroughout the organization to ensure effective and efficientpatient care delivery and the achievement of desired patientoutcomes Obtains and maintains competencies in med/surg, med/tel,ICU, ED, and postpartum/newborn care Accepts assignments to anyarea in which competencies are documented and is able to assistwith nursing function in any department or area of need EssentialFunctions Ability to practice nursing as described by the followingdefinition: The diagnosis and treatment of human responses toactual or potential health alterations. Ability to utilize thenursing process in the provision of nursing care including but notlimited to administering treatments and medications, interpretingdiagnostic tests, formulating a plan of care, collaborating withother health care providers and providing education to patientsand/or significant others. The successful candidate will berequired to pass competencies for this position. Involvesdiscretion and independent action within prescribed limits.

Collects patient health data Determines the priority of datacollection based on the patient’s immediate condition or needsCollects pertinent data using appropriate assessment techniquesInvolves the patient, significant others, and health care providersin data collection when appropriate Ensures that the datacollection process is systematic and ongoing Documents relevantdata in the medial record according to hospital standards Analyzesthe assessment data in determining diagnoses/patient problemsDerives diagnoses/patient problems from the assessment dataValidates diagnoses/patient problems with the patient, significantother, and health care providers, when possible Documentsdiagnoses/patient problems in a manner that facilitates thedetermination of expected outcomes and plan Identifies expectedoutcomes individualized to the patient Derives outcomes from thediagnoses/patient problems Documents outcomes as measurable goalsFormulates outcomes mutually with the patient and health careproviders, when possible Ensures outcomes are realistic in relationto the patient’s present and potential capabilities Ensuresoutcomes are attainable in relation to resources available to thepatient Ensures outcomes include a time estimate for attainmentEnsures outcomes provide direction for continuity of care.

Anticipates potential problems situations and intervenes to offsetany adverse impact Develops a plan of care that prescribesinterventions to attain expected outcomes Individualizes the planto the patient’s condition or need Develops the plan with thepatient, significant others, and health care providers, whenappropriate Ensures the plan reflects current nursing practiceDocuments the plan Provides continuity of care in the planImplements the interventions identified in the plan of careEstablishes interventions that are consistent with the plan of careImplements interventions in a safe and appropriate manner Documentsinterventions Evaluates the patient’s progress toward attainment ofoutcomes Ensures evaluation is systematic and ongoing Documentspatient’s responses to interventions Evaluates the effectiveness ofinterventions in relation to outcomes Utilizes ongoing assessmentdata to revise diagnoses/patient problems, outcomes, and the planof care, as needed Documents revisions in diagnoses/patientproblems, outcomes, and the plan of care Involves the patient,significant others, and health care providers in the evaluationprocess, when appropriate Participates in quality-of-careactivities as appropriate to include but not limited to:

Identification of aspects of care important for quality monitoringIdentification of indicators used to monitor quality andeffectiveness of nursing care Collection of data to monitor qualityand effectiveness of nursing care Analysis of quality data toidentify opportunities for improving care Formulation ofrecommendations to improve nursing care or patient outcomesImplementation of activities to enhance the quality of nursingpractice Utilizes the results of quality-of-care activities toinitiate changes in practice Other duties as assignedQualifications Education Graduate of an accredited school ofprofessional nursing Licenses/Certifications Licensure in the Stateof Ohio as a Registered Professional Nurse (RN) ExperiencePreferred 3-5 years hospital experience Proficient with computersystems and oral and written communication skills
