Job Summary Responsible for directing and coordinatingall nursing care based on established clinical nursing practicestandards. Collaborates with other professional disciplines toensure effective and efficient patient care delivery and theachievement of desired patient outcomes. Essential Functions
- Ability to practice nursing as described by the followingdefinition: The diagnosis and treatment of human responses toactual or potential health alterations.
Ability to utilize thenursing process in the provision of nursing care including but notlimited to administering treatments and medications, interpretingdiagnostic tests, formulating a plan of care, collaborating withother health care providers, and providing education to patientsand/or significant others. The successful candidate will berequired to pass competencies for this position. Involvesdiscretion and independent action within prescribed limits.
- Collects patient health data
- Determines the priority of datacollection based on the patient’s immediate condition or needs.
- Collects pertinent data using appropriate assessment techniques.
- Involves the patient, significant others, and health care providersin data collection when appropriate.
- Ensures that the datacollection process is systematic and ongoing.
- Documents relevantdata in the medial record according to hospital standards.
- Analyzes the assessment data in determining diagnoses/patientproblems.
- Derives diagnoses/patient problems from theassessment data.
- Validates diagnoses/patient problems with thepatient, significant other, and health care providers, whenpossible.
- Documents diagnoses/patient problems in a manner thatfacilitates the determination of expected outcomes and plan.
- Identifies expected outcomes individualized to the patient.
- -Derives outcomes from the diagnoses/patient problems.
- Documentsoutcomes as measurable goals.
- Formulates outcomes mutually withthe patient and health care providers, when possible.
- Ensuresoutcomes are realistic in relation to the patient’s present andpotential capabilities.
- Ensures outcomes are attainable inrelation to resources available to the patient.
- Ensures outcomesinclude a time estimate for attainment.
- Ensures outcomes providedirection for continuity of care. Anticipates potential problemssituations and intervenes to offset any adverse impact.
- Developsa plan of care that prescribes interventions to attain expectedoutcomes.
- Individualizes the plan to the patient’s condition orneed.
- Develops the plan with the patient, significant others, andhealth care providers, when appropriate.
- Ensures the planreflects current nursing practice.
- Documents the plan.
- Providescontinuity of care in the plan.
- Implements the interventionsidentified in the plan of care.
- Establishes interventions thatare consistent with the plan of care.
- Implements interventions ina safe and appropriate manner.
- Documents interventions.
- Evaluates the patient’s progress toward attainment of outcomes.
- -Ensures evaluation is systematic and ongoing.
- Documents patient’sresponses to interventions.
- Evaluates the effectiveness ofinterventions in relation to outcomes.
- Utilizes ongoingassessment data to revise diagnoses/patient problems, outcomes, andthe plan of care, as needed.
- Documents revisions indiagnoses/patient problems, outcomes, and the plan of care.
- Involves the patient, significant others, and health care providersin the evaluation process, when appropriate.
- Participates inquality-of-care activities as appropriate to include but notlimited to:
- Identification of aspects of care important forquality monitoring.
- Identification of indicators used to monitorquality and effectiveness of nursing care.
- Collection of data tomonitor quality and effectiveness of nursing care.
- Analysis ofquality data to identify opportunities for improving care.
- Formulation of recommendations to improve nursing care or patientoutcomes.
- Implementation of activities to enhance the quality ofnursing practice.
- Utilizes the results of quality-of-careactivities to initiate changes in practice.
- Other duties asassigned. Qualifications Education
- Bachelor’s Degree preferred
- Graduate of an accredited school of professional nursing.
Liscenses/Certifications
- Licensure in the State of Ohio as aRegistered Professional Nurse (RN). Experience
- Minimum of oneyear of hospital clinical experience preferred
