Job Summary Responsible for directing andcoordinating all nursing care based on established clinical nursingpractice standards. Collaborates with other professionaldisciplines to ensure effective and efficient patient care deliveryand the achievement of desired patient outcomes. EssentialFunctions

  • Ability to practice nursing as described by thefollowing definition: The diagnosis and treatment of humanresponses to actual or potential health alterations.

Ability toutilize the nursing process in the provision of nursing careincluding but not limited to administering treatments andmedications, interpreting diagnostic tests, formulating a plan ofcare, collaborating with other health care providers, and providingeducation to patients and/or significant others. The successfulcandidate will be required to pass competencies for this position.

Involves discretion and independent action within prescribedlimits.

  • Collects patient health data 1.

1. Determines thepriority of data collection based on the patient’s immediatecondition or needs. 2. Collects pertinent data using appropriateassessment techniques. 3. Involves the patient, significant others,and health care providers in data collection when appropriate. 4.Ensures that the data collection process is systematic and ongoing.5. Documents relevant data in the medial record according tohospital standards.

  • Analyzes the assessment data in determiningdiagnoses/patient problems. 1. 1. Derives diagnoses/patientproblems from the assessment data. 2. Validates diagnoses/patientproblems with the patient, significant other, and health careproviders, when possible. 3. Documents diagnoses/patient problemsin a manner that facilitates the determination of expected outcomesand plan.
  • Identifies expected outcomes individualized to thepatient.

1. 1. Derives outcomes from the diagnoses/patientproblems. 2. Documents outcomes as measurable goals. 3. Formulatesoutcomes mutually with the patient and health care providers, whenpossible. 4. Ensures outcomes are realistic in relation to thepatient’s present and potential capabilities. 5. Ensures outcomesare attainable in relation to resources available to the patient.6. Ensures outcomes include a time estimate for attainment. 7.Ensures outcomes provide direction for continuity of care.

Anticipates potential problems situations and intervenes to offsetany adverse impact.

  • Develops a plan of care that prescribesinterventions to attain expected outcomes. 1. 1. Individualizes theplan to the patient’s condition or need. 2. Develops the plan withthe patient, significant others, and health care providers, whenappropriate. 3. Ensures the plan reflects current nursing practice.4. Documents the plan. 5. Provides continuity of care in the plan
  • Implements the interventions identified in the plan of care. 1.1. Establishes interventions that are consistent with the plan ofcare. 2. Implements interventions in a safe and appropriate manner.3. Documents interventions.
  • Evaluates the patient’s progresstoward attainment of outcomes.

1. 1. Ensures evaluation issystematic and ongoing. 2. Documents patient’s responses tointerventions. 3. Evaluates the effectiveness of interventions inrelation to outcomes. 4. Utilizes ongoing assessment data to revisediagnoses/patient problems, outcomes, and the plan of care, asneeded. 5. Documents revisions in diagnoses/patient problems,outcomes, and the plan of care. 6. Involves the patient,significant others, and health care providers in the evaluationprocess, when appropriate.

  • Participates in quality-of-careactivities as appropriate to include but not limited to: 1.

1.Identification of aspects of care important for quality monitoring.2. Identification of indicators used to monitor quality andeffectiveness of nursing care. 3. Collection of data to monitorquality and effectiveness of nursing care. 4. Analysis of qualitydata to identify opportunities for improving care. 5. Formulationof recommendations to improve nursing care or patient outcomes. 6.Implementation of activities to enhance the quality of nursingpractice. 7. Utilizes the results of quality-of-care activities toinitiate changes in practice.

  • Other duties as assigned.

Qualifications Education

  • Bachelor’s Degree preferred
  • Graduateof an accredited school of professional nursing.

Liscenses/Certifications

  • Licensure in the State of Ohio as aRegistered Professional Nurse (RN). Experience
  • Minimum of oneyear of hospital clinical experience preferred
