Overview:

We’re looking for a Hospice Nurse Practitioner to join ourteam. You will report directly to the Executive Director orAdministrator. You will be assigned to a location to performFace-to-Face (F2F) encounters and be responsible for gatheringclinical information to support the hospice diagnosis andprognosis. These findings are then communicated to the hospicephysician responsible for certifying or recertifying the patient’shospice eligibility. The scope of practice for the hospice NP islimited to those tasks necessary to complete hospice F2F encountervisits. Performs and completes physical assessments for the F2Fencounter within the regulatory time frames established by CMS Provides clinical findings, including review of the medicaltreatment plans, diagnostic tests, lab results, and medicationlists to the physician who is recertifying the patient for hospiceeligibility Makes appropriate recommendations to the IDG based on findingsof the F2F assessment Notifies hospice agency when unable to complete a F2F visitaccording to schedule

About You:

Master’s or higher degree in Nursing. Completion of a graduate-level nurse practitioner educationprogram accredited by a national accrediting body. Current state license as a registered nurse Current state license or certificate as an advanced practicenurse Current national certification as a Nurse Practitioner. (ANCCor AANP) Must have a collaborative agreement in place.
