Job SummaryThe RN Case Manager is responsible to facilitate carealong a continuum through effective resource coordination to helppatients achieve optimal health, access to care and appropriateutilization of resources, balanced with the patient’s resources andright to self-determination. The individual in this position hasoverall responsibility for ensuring that care is provided at theappropriate level of care based on medical necessity and to assessthe patient for transition needs to promote timely throughput, safedischarge and prevent avoidable readmissions. This positionintegrates national standards for case management scope of servicesincluding:

  • Utilization Management supporting medical necessityand denial prevention
  • Transition Management promotingappropriate length of stay, readmission prevention and patientsatisfaction
  • Care Coordination by demonstrating throughputefficiency while assuring care is the right sequence and atappropriate level of care
  • Compliance with state and federalregulatory requirements, TJC accreditation standards and Tenetpolicy
  • Education provided to physicians, patients, families andcaregivers Job ResponsibilitiesThe individual’s responsibilities include thefollowing activities: a) accurate medical necessity screening andsubmission for Physician Advisor review, b) care coordination, c)transition planning assessment and reassessment, d) implementationor oversight of implementation of the transition plan, e) leadingand facilitating multi-disciplinary patient care conferences, f)managing concurrent disputes, g) making appropriate referrals toother departments, h ) identifying and referring complex patientsto Social Work Services, I) communicating with patients andfamilies about the plan of care, j) collaborating with physicians,office staff and ancillary departments, k) leading and facilitatingComplex Case Review, l) assuring patient education is completed tosupport post-acute needs , m) timely complete and concisedocumentation in Case Management system, n ) maintenance ofaccurate patient demographic and insurance information, o)identification and documentation of potentially avoidable days, p)identification and reporting over and underutilization, q) andother duties as assigned
EducationRequired:

Graduate of an accredited school of nursing.

Preferred:

Academic degree in nursing (bachelor’s or master’s)

ExperienceRequired:

2 years of acute hospital patient careexperience.

Preferred:

Acute hospital case managementexperience.

CertificationsRequired:

RN. Must be currently licensed, certifiedor registered to practice profession as required by law orregulation in state of practice or policy. Active RN license forstate(s) covered.
