$10,000 Sign On Bonus Job Summary TheClinical Care Leader (CCL) is responsible for utilizing the nursingprocess (assessment, diagnosis, outcomes/planning, implementationand evaluation) to provide individualized nursing care in the homesetting. Collaborates with other inter-professional colleagues, toplan, implement and evaluate care. Documents resident history,symptoms, medications and care provided in compliance withapplicable standards/regulations. Utilizes technology to enhancequality resident care, to collaborate with inter-professional teammembers, effectively delegate to and supervise unlicensedpersonnel, assume overall nursing accountability for a residentassignment and serve in leadership roles ondepartment/organizational tasks forces and committees. Demonstratescompetency and practices within the full scope of nursingexpertise/knowledge and utilizes appropriate age and populationspecific standards as designated in the specific care area or unit.
Must have effective communication skills to collaborate, negotiateand resolve conflicts, situations, and/or issues with peers,inter-professional colleagues, residents and families. Must havestrong clinical reasoning and critical thinking skills to analyzedata and appropriately respond to changing situations. coordinationof care, resident assessment, resident education, triage, andvarious other nursing interventions. The Sanford Professional Nursing Practice recognizes the Scope andStandards of Practice and the Code of Ethics for Nurses withInterpretive Statements as published by the American NursesAssociation as the foundation of nursing care delivery andprofessional conduct. Qualifications Bachelor’s degree in nursingpreferred. Graduate from a nationally accredited nursing programpreferred, including, but not limited to, Commission on CollegiateNursing Education (CCNE), Accreditation Commission for Education inNursing (ACEN), and National League for Nursing Commission forNursing Education Accreditation (NLN CNEA). Minimum of two years’long term care or geriatric nursing experience preferred. MinimumData Set (MDS) experience preferred but not required. Currentlyholds an unencumbered RN license with the State Board of Nursingwhere the practice of nursing is occurring and/or possessmultistate licensure if in a Nurse Licensure Compact (NLC) state.
Obtains and subsequently maintains required department specificcompetencies and certifications. Depending on location, a validdrivers license may be required. Basic Life Support (BLS) required.
Resident Assessment Instrument (RAI) process experience preferred,but not required.
