POSITION SUMMARY
The Nurse Navigator for Congestive Heart Failure (CHF) serves as a patient advocate, educator, and coordinator, guiding patients through the healthcare system to ensure seamless, high-quality care. This role focuses on improving patient outcomes by providing education, support, and coordination of care for CHF patients, their families, and healthcare providers.
POSITION QUALIFICATIONS
Minimum Education Associate Degree in Nursing with relevant experience Minimum Experience 2-3 years of clinical nursing experience, preferably in cardiology, ehart failure management, or case management. Required Registration/License/Certification Active Registered Nurse (RN) license in Alabama Basic Life Support (BLS) certification
