While prior nephrology experience is not required, a strong understanding of chronic conditions is essential, as our patient population primarily consists of elderly individuals with complex medical histories. The ideal candidate will be committed to providing high-quality, patient-centered care and will serve as a key liaison between patients and physicians. Key Responsibilities Triage nurse phone calls, providing timely and appropriate clinical guidance. Manage prescription refills and prior authorization requests efficiently. Complete medical paperwork including forms for insurance, disability, and specialist referrals. Act as a bridge between patients and providers, facilitating clear communication and coordinated care. Educate patients and their families on disease processes, medication regimens, and health maintenance strategies. Monitor and document patients’ vital signs, medical histories, and ongoing care needs. Maintain accurate and up-to-date medical records in compliance with HIPAA and office protocols. Qualifications Current Registered Nurse (RN) license in good standing. Familiarity with chronic conditions such as hypertension, diabetes, and kidney disease.
